  • Back in April 2021, we requested that employees send the District copies of their Vaccination Record Cards. Thank you to the many employees who have already sent the information. If you have not already done so, please send your vaccination record card to HR as soon as possible.
    By collecting this information, the District may be able to limit any potential quarantines that you otherwise may face in the future. For instance, if you are determined to be a close contact of a person who has tested positive or is presumed to be positive for Covid-19, you are not required to quarantine as long as you remain asymptomatic and you have provided a copy of your COVID-19 Vaccination Record Card to Human Resources. If you do not provide a vaccination record card in advance, your ability to remain at work or return to work may be delayed resulting in unnecessary use of your personal leave bank. 
    Vaccination cards should be sent once you are fully vaccinated (two doses of Pfizer or Moderna or one dose of Johnson and Johnson). Please do not send the original Record Card. Also, if your social security number is listed on the Record Card, please redact your social security number on the copy prior to sending it to Human Resources. Copies of the Record Card can be sent through interoffice mail, dropped off in a sealed envelope to the Administration Building, sent by mail or scanned and emailed to
