1. Age Verification – hospital, birth, or baptismal certificate
Note: Students must be 5 years old by September 1st to enter kindergarten and 6 years old by September 1st to enter the 1st grade. No exceptions will be granted.
2. Proof of Immunization – written immunization record signed by a physician as proof of the following:
- 4 doses of Tetanus and Diphtheria with the 4th dose on or after the 4th birthday (usually given as DPT, DT, or TD vaccine)
- 2 doses of Measles, Mumps, and Rubella, first given after the 1st birthday (usually given as MMR)
- 3 doses of Polio vaccine (if given as OPV) or 4 doses of IPV
- 3 appropriately spaced doses of Hepatitis B (usually given as Hep B, H-BTG, Recombivax, or Energix)
- 1 dose of Varicella vaccine (or documentation of Chicken Pox illness)

3. Proof of Residency – (each of the following items must be presented)
- mortgage, deed, tax bill or rental agreement
- current utility bill
- valid Pennsylvania driver’s license/state identification card.
All documents must reflect your current address. If you are supplying an unlisted or cell phone number, you will also be required to provide your phone bill. Additional documents may be required based on your residency status.

6. Parental Registration Statement – Please click here to access and print this form. This form is required for all registrations of students in grades 1 through 12. The parent/guardian must complete the information, and have the form notarized.
